How you can help PPNP take better care of children


In December 2010 our practice became a Certified Medical Home. A Medical Home is an approach to healthcare with the emphasis on a partnership between families and pediatric clinicians. With parent feedback, it improves medical care to children with special healthcare needs. This partnership enhances continuity, empowers families to make decisions, and enables the healthcare providers to learn from each experience to help better serve patients.

The process starts by getting feedback from families and patients about how good (or bad) a job we are doing. This is done through an anonymous survey distributed by the Pennsylvania Medical Home Initiative. If you have a child with special health care needs or a chronic condition and wish to participate in the survey all you need to do is fill out the “Authorization for Disclosure of Protected Health Information” and return it to your child's healthcare provider. Once this form is completed your child will become part of our “Medical Home Registry” and you will receive a survey from the “Pennsylvania Medical Home Initiative” in which you rate our practice in many areas including accessibility, parent satisfaction, health care utilization, and unmet medical needs. This survey is confidential and will help us learn where we can improve as a practice and better serve our patients.
